4. Whether of pure European descent?

5. Whether a British subject by birth or naturalization. (State which, and if by naturalization attach a certificate from the Home Office)

6. Nationality by birth of father (if naturalized, state date)

7. Nationality by birth of mother (if naturalized, state date)

8. Nationality by birth of husband (if married)

9. Occupation of father

10. Permanent address of Candidate

11. Present address for correspondence

12. Schools or Colleges at which educated, and any degrees or diplomas obtained

13. Occupation or employment in civil life

14. Have you any special qualifications or experience which, in your opinion, qualify you for the appointment asked for?

15. Whether now serving, or previously served, in any other Government Dept. (Home, Indian or Colonial)? If so, give particulars, and attach official permission from the head of the Dept. to make this application
