1. I declare that I am a British subject, and that I will be faithful and bear true allegiance to His Majesty King George V., his heirs and successors.

2. I declare my loyalty to the .... and promise obedience to the Commandant and Officers of my detachment. I agree to obey and be bound by all rules and regulations from time to time passed by the Joint V.A.D. Committee, and I agree that I am liable to dismissal by my Commandant subject to the approval of my County Director without reason assigned.

3. I engage to devote sufficient time to rendering myself efficient, and I undertake to give one month's notice of my intention to resign.
